This website provides general information about DJL Fitness HRT. It does not operate a patient portal, clinical record, or embedded patient-intake system. The site guide accepts only fixed navigation choices and does not transmit or save a visitor's response.
Information handled by the website
The static hosting and security provider may process ordinary technical request information such as IP address, browser details, requested page, timestamps, and security events to deliver and protect the website. The site does not include advertising pixels, session replay, or third-party analytics.
Do not submit medical information
Do not send symptoms, diagnoses, medications, lab results, insurance information, medical history, treatment history, or other private medical information through the public site or general email. Use only the separate secure clinical workflow when instructed by the practice.
Online scheduling and external services
Appointment links open the practice's MyPatientVisit scheduling page in a new tab. Information entered there goes directly to that service and does not pass through this public website. Follow the practice's instructions for clinical intake.
CareCredit financing and brand-information links also open external websites. Review those services' privacy notices and terms before using them. The public site does not embed their forms or tracking tools.
